Why did my breakup happen after 14 great months?

My ex and I broke up in mid-July after 14 mostly wonderful months together. The last six weeks however devolved into an exhausting cycle of defensive arguments where neither of us felt heard. While there was no abuse infidelity or name-calling we were stuck in a pattern of fighting to defend ourselves rather than listening.
During this exact timeframe my personal life hit rock bottom. My aunt was dying I lost my job my new job's start date was repeatedly delayed my finances blew up my living situation was draining and my parents were divorcing. I was severely overwhelmed and emotionally reactive without fully realizing it at the time.
The breaking point happened on her birthday:
She had planned a hibachi dinner and bar night with friends.
A female friend of mine asked for a ride. I asked my girlfriend if this friend could join dinner; she explicitly said no.
When we arrived my friend unexpectedly decided to walk in with me anyway. Instead of setting a boundary I stupidly convinced myself it would be fine since they knew each other.
My girlfriend was rightfully furious. Rather than comforting her when she ended up outside in tears I got defensive felt misunderstood and left.
The next day during another heated argument I told her "I don't know how you're going to react and manage when life presents you with true challenges." I deeply regret saying that.
Although I apologized days later the relationship never recovered. We fell into a loop where every subsequent fight circled back to her birthday. She suggested a break but I pushed to keep working through it. Three weeks later we broke up.
Since then therapy has opened my eyes. I realize now that I was prioritizing explaining my "innocent intentions" over acknowledging her hurt. Bringing that friend after she explicitly said no was disrespectful period. My personal crisis explained my emotional state but it didn't excuse my behavior.
After three weeks of no-contact I sent her a sincere letter of accountability with no expectation of a reply. Five weeks later I asked if she wanted to grab ice cream. She politely declined stating she thinks it’s best we move forward separately. I am fully respecting her decision and will not reach out again.
I accept why we needed to end but I’m struggling to make sense of the damage.

If I were her, yes I would hear you out. I think it’s reasonable. And you don’t seem like you had any malicious intent, it just seems like you were overwhelmed and I’m sure it felt really awkward to try to tell your other friend that she wasn’t invited. So I honestly understand why that situation happened, it was not an easy one to handle.
